Preregistration for MAICS-96

Please fill out and send the following form. Your registration cannot be processed without at least your name and a valid credit card number. When you click on Send, it will be sent to our Registrar at the IU Conference Bureau (cbtech@indiana.edu). A receipt message will be returned to you, and a hardcopy receipt will be available at the registration table at the conference.

Please note that this fee is for conference registration including Friday and Saturday evening social events and other refreshments, but does not address any housing needs. One day registration is not available.
